Navigator Workflow

Navigator Workflow is a patient-centered navigation module that aligns your organization’s patient navigation goals (e.g. readmission reduction, service line growth, etc.) with the optimal navigational path for each patient. Unlike native navigation functionality in your EMR, Navigator Workflow transforms a complex patient journey into a series of discreet steps.

Navigator Predict

The impact of Care Continuity’s software starts with Navigator Predict, the machine learning engine that prioritizes patients for navigation based on your program goals, network dynamics, and patient characteristics. When a patient is discharged from your emergency department, discharged from an inpatient admission, or receives a specialist referral from their PCP, they enter the Navigator Predict pool. Navigator Predict ingests information from the triggering encounter, including patient engagement and clinical history, and details of your provider network dynamics.

These details are combined with over 50 weighted navigation variables and a prediction score is created through machine learning. The prediction score represents a higher confidence for a successful patient navigation. Meaning, a higher score is a patient most in need of navigation and most likely to engage with navigational help. The patients then enter into the appropriate queue based on their prediction score.

While all patients who enter the Navigator Predict pool receive navigation, the prediction score allows for better prioritization and better program results for your health system.
