I have an internal patient navigation team, why should I use Care Continuity instead?

Internal patient navigation teams can be a good option for health systems that want to use their current resources to help patients get to their next step in care. However, the limitations of this approach prevent scalability and hinder noticeable returns in investment.

The key to Care Continuity’s success in navigating over two million patients lies in our supervised machine learning models that both prioritizes and predicts patients not only most in need of navigation, but also most likely to accept assistance from a patient navigator.

This means that time is not spent attempting to navigate every patient, but instead the focus is put on the ones most likely to return.

For health systems that have made significant investments in their internal patient navigation team, they can still utilize Care Continuity’s software to optimize their results.

Simply put, your EMR (no matter which one) was not designed to meet the demands of a mature patient navigation program. This fact becomes readily more apparent as the scale and complexity of your program increases. To be sure, your organization will need to navigate 2.5 to 3.6 MORE patients to get the same results as you would with an intelligent, data-driven solution like the Care Continuity platform. Without the proper tools, most health systems simply hire more FTEs to achieve their goals. Unfortunately, most health systems come to this realization only after several months or years of trial, error and frustration.

Care Continuity’s Navigation Solution Suite is a purpose-built system designed to match the requirements of varied patient populations, reimbursement structures (FFS, VBC and everything in-between) and provider networks. Our technology is informed by over 10 years of navigation with millions of patients navigated for hundreds of hospitals and other healthcare organizations to thousands of primary care providers and specialists.

A well-designed patient navigation program can drive significant impact on many of the main goals of value-based care.

One of the main impacts patient navigation has on value-based care agreement and quality goals is the improvement on health outcomes. With active navigation, patients are 30 – 50% less likely to be readmitted after an IP stay or to have repeat visits to your emergency room. In addition, our ability to prioritize and target the highest-risk patients enables us to focus navigational resources on those most in need of assistance.

In addition, a high-quality patient navigation program can greatly impact patient experience metrics. With patient navigators making scheduling appointments easier, reducing median time to appointments, and helping throughout their care journey, patients benefit from a comprehensive patient navigation program.

While capacity constraints are a big issue faced by many healthcare providers today, Care Continuity’s patient navigation program is customizable to the specific needs of any health systems, including those who are facing capacity issues.

Care Continuity’s software is able to load balance patients and direct them to the appropriate next step in their care journey while working around capacity issues. In fact, through in-depth data reporting, Care Continuity can uncover potential areas of improvements that can alleviate capacity issues.

When navigating patients with Care Continuity, health systems unlock a suite of analytics and data dashboards that can help them better manage downstream care and improve system operations.

Health systems gain access to valuable insights into their network performance. Depending on the program, this can include appointment completion rates, median time to appointment, in-network rates, appointment-to-procedure rates, and more. In addition, these statistics can be viewed at granular levels such as by office locations, payer types, service lines, and other metrics and demographics.

Care Continuity understands the importance of the navigator in running a successful navigation program. We take great care in hiring and training our patient navigation team to provide the best possible experience for both our health system clients and their patients.

All Care Continuity patient navigators are trained using our best practices developed over our more than a decade experience navigating more than two million patients for some of the nation’s leading health systems.

Our navigators are led by seasoned healthcare veterans who know the intricacies of today’s healthcare landscape as well as the goals of each client health system.
